What's an Advance Healthcare Directive?
An Advance Healthcare Directive is an important part of any estate plan. It is also known as a Living Will. This document gives instructions to your medical team and your loved ones in regards to your wishes if you become incapacitated and cannot make medical decisions for yourself. It is never comfortable talking about what you would like to happen if you are struck ill and cannot communicate your wishes, however, know that your written instructions are a gift to your loved ones.
An Advance Healthcare Directive is used only when you are incapacitated and cannot make medical decisions for yourself. It has provisions for medical treatment, selection of healthcare providers, diagnostic tests and procedures, administration or withholding of artificial nutrition and hydration, CPR and anatomical gifts. You may specify in your Advance Healthcare Directive what your wishes are so that your agent(s) can follow your instructions.

due to strict privacy laws in regard to our health information. By signing this form, you will be able to also give your agent(s) access to your protected health information in order for them to work with doctors, government agencies or others in order to give you the best care.
